Abstract

A 23-year-old female presented with gradual, painless DOV in OS since 1 month. BCVA was 6/6 OD and 1/60 OS. Anterior segment OU was normal. Fundus OS revealed a large (2 DD by 3 DD) juxta-papillary, yellowish-white sub-retinal lesion, adjacent to disc with calcification encroaching fovea & shallow SRF. SD-OCT showed a large choroidal hyporeflective space with RPE degeneration and atrophy with multiple intra-retinal cystic pockets. The lesion showed early patchy choroidal hyperfluorescence with late diffuse staining on FFA.ICGA showed early corresponding hypocyanescence, with late diffuse multifocal hypercyanescence. B-Scan USG showed pseudo-optic nerve sign. Detailed systemic investigations (including MRI brain, USG abdomen, RFT, dermatology consult) to rule out phakomatoses were all normal. She received two consecutive monthly intravitreal injections of Aflibercept. Post injection evaluation revealed resolution of intra-retinal cystic spaces on SD-OCT, with stabilization of vision.
