Abstract
We are reporting 2 cases who received Neo-adjuvant chemotherapy (NACT) [5-fluorouracil (5-FU) and cisplatin] for sebaceous gland carcinoma (SGC) of eyelid, and who developed serious corneal complications. Both had no predisposing ocular surface disorder. One patient developed a small sterile corneal melt while the second developed a complete sterile corneal melt. Both patients had large upper eyelid SGC with mechanical ptosis which showed a dramatic response to NACT. 5-FU is known to cause inhibition of the mitotic activity of corneal epithelial cells and hence increases the risk of corneal epithelial defects. However, significant corneal ulcerations have not been reported earlier with systemic use for either non-ocular or eyelid malignancies. We hypothesize tumour releasing inflammatory mediators during necrosis to be the cause . Physicians using systemic NACT for upper eyelid tumours should be aware of this potential sight-threatening complication to identify it early.
