Abstract

Accurate biometry is the key to good visual outcome in cataract surgery.Despite use of optical biometers,dense cataracts are challenging.We report a case of mature cataract with posterior staphyloma detected on B-scan, with axial length not recordable on optical biometer and variable readings on immersion A scan.We performed a Cataract extraction first and an Optical Biometry was done in Aphakic Mode.Using preoperative keratometry readings, IOL power was calculated and implanted on the same day.At One month follow up ,the Best corrected Visual Acuity was 6/24 not improving further due to Myopic retinal degeneration.The spherical equivalent of the correction on Autorefractometer was +0.75 D, which implied a fairly accurate IOL power calculation with this method.If there is a wide variation in the measurements of AL, this technique of biometry on a non-contact optical biometer immediately after cataract extraction, and a secondary IOL implantation can be considered.
