Abstract
Aim:To establish role of anterior segment optical coherence tomography (ASOCT) in guiding treatment, in cases of corneal edema with Descemet’s membrane (DM) anomaly. Methods:Retrospective analysis over 2 years. Results:Fourteen cases of unilateral corneal edema with vision CF-3m or less were included. DM tear/detachment was seen in 7 cases (4=hydrops,4=post cataract surgery DMD). C3F8 descemetopexy was done in 6 cases, 1 case had self resolution and keratoplasty was planned in one. Four cases had corneal edema post-phaco; OCT showed increased thickness (>800) with multiple DM folds; managed conservatively on medical therapy. In 2 cases OCT showed endothelial KPs confirming diagnosis of herpesviral keratitis; managed with anti-viral therapy. Conclusions:Severe corneal edema results in poor visibility of posterior stroma, DM and anterior chamber on slit lamp examination. ASOCT plays a significant role in determining presence of DM anomalies in such cases and guides treatment strategies.
