Abstract
A 75-year female presented with an ulcer of right lower eyelid for one year. She had a basal cell carcinoma at the same site 25 years back for which she underwent excision with skin graft and eight rounds of radiotherapy of total 30 Gray dose over a period of 4 weeks elsewhere. On examination, there was a single ulcer of size 3cm×3cm with rolled out edges, irregular margin without any bleeding or discharge, not fixed to any underlying structure &no regional lymphadenopathy. Routine haemogram & peripheral smear was within normal limit. Histopathological examination revealed nests of uniform basoid cells with palisading arrangement suggestive of basal cell carcinoma. MRI brain & orbit showed diffuse fatty infiltration of skull bones and subtle plaque like thickening along the right inferior lid. Wide excision& full thickness skin graft was performed with Mustarde cheek rotational flap. Though recurrence in a rodent ulcer is rare, it may occur following inadequate treatment.
