Abstract

Aim-To describe the novel use of cyanoacrylate glue for successful management of pediatric Pythium keratitis. Methods-A 9-year-old young boy presented with pain, redness, and defective vision in the RE post stick injury. BCVA was 20/40 on presentation, which deteriorated to HM within 5 despite treatment.Anterior segment examination revealed a 6×5 mm dry looking mid stromal infiltrate with feathery margins.Corneal scraping and culture were performed, and the patient was treated with antifungals. Results- Despite antifungals for 5 days, the infiltrate progressed.Hence Pythium was kept as differential, which was confirmed on culture. The patient was started on 0.2% Linezolid and 1% Azithromycin hourly.Due to the rapid progression of infiltrating, corneal melt, and younger age, cyanoacrylate glue and BCL were used.On last follow-up, the BCVA was 20/40. Conclusion-Cyanoacrylate has antibacterial properties, act in synergy with antibacterialS, and can be a potential rescuer in these cases.
