Abstract
56 yrs female presented with left medial canthal swelling of 3 yrs,with mild epiphora.No H/O dacryocystitis, facial trauma,surgery or discharge. On examination,non tender,non mobile, subcutaneous mass at medial canthus of 4×3.5x2cm.Syringing showed blockage of nasolacrimal duct.CT scan showed non-enhancing,low-density, well defined,thin walled cystic lesion at medial orbital margin.No signs of bony erosion,consistent with idiopathic acquired dacryocystocele.Patient underwent aspiration of pus (15ml) followed by external DCR.Aspirated sac fluid was mucopurulent & did not reveal any growth. Patient’s symptoms of epiphora resolved after surgery.
Dacryocystocele refers to diffuse, centrifugal enlargement of the lacrimal sac resulting from combined proximal & distal obstructions in lacrimal drainage system.Its presence in adults is extremely rare event.In adults, differential diagnosis of mimicking dacryocystitis, diverticula, malformations & neoplasms must be kept in mind.
