Abstract

42yr old male farmer presented with forward bulging, lid swelling, pain,diminished vision right eye since 3 days following trauma by vegetative matter.On examination RE abaxial proptosis,vertical dystopia, raised IOP,marked restriction of movement, superotemporal lid fullness,edema,erythema ,tenderness, with normal overlying skin,mechanical ptosis ,presence of crepitus and lagophthalmos, conjunctival chemosis, corneal epithelial defect was noted. Investigation done including B.scan,CT scan confirmed trapped air within loose subcutaneous tissue in orbit.Patient was though planned for RE emergency lateral Orbitotomy under GA, but lateral canthotomy under LA was done, since patient was not willing. Medical treatment with eye drops (lubricant ,antibiotic,IOP lowering agents)and injectable and oral steroids were given. Over follow up visits every fortnightly ,patient improved symptomatically in terms of vision ,proptosis, vertical dystopia, IOPand movement.
