Abstract

Purpose: To describe a case of traumatic subluxation of Bigbag IOL. Methods: A 25-year-old female presented with complaints of sudden painless diminution of vision in left eye associated with a whitish reflex in the pupil following alleged trauma to left eye 10 days ago. She had history of poor vision since childhood and was diagnosed as having bilateral cataract. She had undergone cataract surgery with intraocular lens (IOL) implantation in 2014 in both eyes with subnormal gain of vision. Vision in right and left eye was 6/36 and 4/60 respectively. In left eye, a Bigbag IOL was found subluxated, with one haptic in anterior chamber and optic in the bag. Fundus showed bilateral retinitis pigmentosa. Results: Explantation of Bigbag IOL was followed by glued three-piece IOL done in the same setting, under local anesthesia and guarded visual prognosis. Post-op vision improved to 6/60 with well-centered SFIOL. Conclusion: A subluxated Bigbag IOL has been rarely described in literature.
