Abstract

A 62-year-old female, came with complaints of swelling in the right with purulent discharge since 4 months. She was diagnosed as a case of basal cell carcinoma, for which surgical excision was done 3 months ago. On examination, visual acuity was OD : PL -ve and OS: CF 3m. OD was pthytic, medial orbital wall was eroded and orbital bony margins exposed. Purulent discharge was seen within the exposed bony cavity with maggots. MRI showed findings of a mass in the periorbital region with involvement of upper and lower eyelids, preseptal compartment, lacrimal sac and medial rectus. Management included surgical excision and exenteration of secondarily infected basal cell carcinoma along with debridement and lateral temporal forehead flap covering defect over the right side of the face.
