Abstract

Delayed decrease in vision post cataract surgery can be attributed to a multitude of reasons. Opacification of Intraocular lens(IOL) is one among them. It occurs most commonly with hydrophilic IOLs ,and rarely with PMMA ,hydrophobic and silicone IOLs.Our patient an 81 year old female ,who came seven years after cataract surgery with decrease in vision in her left eye(LE). Her anterior segment examination revealed an opacified IOL with a grossly normal posterior segment. Anterior segment OCT revealed hyperreflectivity over anterior surface of IOL .thus we came to the diagnosis of opaque IOL.She underwent a IOL exchange ,and subsequently her vision improved. Misdiagnosing a case of IOL opacification to posterior capsular opacification, and subsequent ND-YAG laser capsulotomy procedures can inadvertently compromise the bag integrity and complicate an IOL exchange procedure. Henceforth proper diagnosis of this condition is necessary.
