Abstract
PURPOSE:Corneal hysteresis(CH) is measure of viscoelastic damping of cornea. Cornea, sclera, peripapillary ring, and lamina cribrosa(LC) are made from the same collagenous matrix. Changes in LC in DR may potentially reflect as changes in CH. The study was undertaken to evaluate CH as biomarker for DR severity, for first time.
METHODS:Tertiary care center-based cross-sectional study. 68 consecutive cases of T2DM were included. No DR(n=22); non-proliferative DR(NPDR)(n=24); proliferative DR(PDR)(n=22) and controls(n=22)[ETDRS]. CH was obtained using Ocular Response Analyzer(USA).
RESULTS:Mean CH: controls=10.88±0.87, no DR=10.25±0.60, NPDR=8.82±0.71 and PDR=8.09±0.76. Low CH correlated significantly with DR severity(p<0.001). On AUC analysis, CH was found to be sensitive and specific for DR severity. CONCLUSIONS:Low CH correlates with DR severity implying corresponding changes in LC affecting neurovascular retinal components. It serves as sensitive and specific indicator for DR severity.
