Abstract
Cataract surgery in posterior polar cataract(PPC) is challenging due to the high incidence of posterior capsular rent(PCR) and nucleus drop. This paper describes novel Slow swirl phaco technique, a safe technique in soft PPCs which avoids turbulence and chances of breach in the posterior capsule. Hydrodissection and nucleus rotation is avoided in this technique. Hydrodelineation is performed. The soft nucleus is then emulsified with a repeated swirling movement of the phaco probe, removing it in layers. Most of the nucleus layes opposite and to the sides of the phaco probe can be removed in this fashion. The phaco parameters are kept low. The central PPC plaque and the sub incisional nucleus can be removed with the irrigation aspiration probe after viscodissection. A study conducted in twenty five patients showed that this is a safe technique in soft posterior polar cataracts with nil intra operative complications.
