Abstract

Most cases of Brown syndrome are reported to be sporadic or non-familial. The incidence of the familial Brown syndrome is rare and possibly of autosomal recessive or dominant inheritance with reduced penetrance. We are reporting a case of right-sided unilateral Brown syndrome in two sisters, 12 and 15 years old who presented with outward deviation and diminution of vision in the right eye since birth as informed by their mother. Other family members were not affected. Exaggerated Guyton’s tendon traction test was strongly positive for superior oblique in the right eye of both patients and superior oblique chicken suture with horizontal muscle strabismus surgery was performed. Patients were orthophoric in the immediate postoperative period and alignment was maintained till 3 months of follow-up. To the best of our knowledge, this is the first report of Familial Brown syndrome from India.
