Abstract
Purpose : To study the clinical and imaging features of lacrimal gland inflammation in patients with granulomatosis polyangiitis and to compare it with those of idiopathic inflammation (idiopathic dacryoadenitis).Materials and Methods: Retrospective, interventional, comparative case series.Results : There were 5 eyes in GPA group and 7 eyes in idiopathic dacryoadenitis group. The mean age group was 35 yrs. The interval from presentation to definite diagnosis range from 1 to 3 months. The most common presenting symptom was lid oedema . Superior rectus involvement was present in all the patients. Sinonasal involvement and bone destruction was absent in all cases. ANCA was positive in three patients. In idiopathic dacryoadenitis the most common muscle involved was lateral rectus. Conclusion: Lacrimal gland inflammation with predominant superior rectus involvement warrants biopsy to rule out GPA even if hematological investigations are negative , especially in younger age group.
