ILM pits with sub retinal fluid have been managed in different ways. Laser to the edge of ONH near the pit area and vitrectomy with ILM peeling with Gas are most commonly practiced with variable degree of success. This video describes a case of 45 yrs lady who presented with Optic disc pit and sub retinal fluid. Other eye was with in normal limits.Visual acuity at presentation was CF 2mts .OCT documentation was done of the same. This video shows vitrectomy with ILM peeling and grating the ILM by inverted flap technique to the optic dis pit and gas tamponade. The results are very promising with good anatomical and functional success.
