Ocular Foreign bodies are not uncommon. A seemingly Large intraocular foreign body documented on USG B-scan and X-ray could not be found even after exploring the presumed impaction site and latter the conformed exit wound through the sclera by leakage of intravitreal air and silicon oil which was sutured with utmost difficulty because of its location just beneath the insertion of lateral rectus. The eye was closed with silicon oil injection and patient is having useful vision in that eye at 6 month of followup without any significant complication. Post operative CT scan showed an orbital foreign body which was left untouched. All is well that ends well..
