Abstract

Blunt trauma has volatile ocular manifestations with crystalline lens extrusion being a rare event resulting from indirect scleral rupture. A 70 year old male presented to the A&E with alleged history of blunt trauma with a wooden shovel handle 3 hours back complaining of sudden painful loss of vision in right eye along with some yellowish mass coming out of the eye. On examination, BCVA in right eye was PL+, PR inaccurate and 6/6 in left eye. Slit lamp examination of right eye revealed conjunctival congestion, cataractous lens stuck in upper fornix, scleral tear from 1CH to 5CH with uveal prolapse, corneal edema with ¾ anterior chamber hyphaema. Posterior segment details of right eye could not be visualised; left eye was WNL. Right eye lens was not visualised in NCCT orbit. Right eye globe exploration with scleral tear repair with AC wash & anterior vitrectomy under GA was done. He was left aphakic to be taken up for secondary IOL implantation later.
