Abstract

Purpose: ICROP 3rd edition defined “Notch” as incursion of ROP lesion into a more posterior zone. In the foetal retina, vasculogenesis proceeds from the optic disc as 4 ellipses in a butterfly wing pattern. By the end of vasculogenesis, the ellipses lose their overlap creating a wedge/ notch in between. Natural course of “notch” is demonstrated by imaging in a preterm baby

Methods: Serial imaging was done using 3nethra Neo camera (Forus Health, India) in a preterm baby with a gestational age 25+6 weeks and birth weight 800 grams.

Result: At 33 weeks post menstrual age (wPMA), ROP without plus was detected with a ridge in zone II and a V shaped notch extending posteriorly into zone I. Gradually, the notch became narrower, started resolving by 39+3 wPMA and was complete by 47+6 wPMA. Ridge regressed by 55+4 wPMA

Conclusions: Serial documentation in this baby helped to understand the evolution of notch in ROP. A notch in zone 1 with rest of the ridge in zone 2 and no plus can be observed
