Abstract

Prevalence of Neurodegenerative diseases is increasing in India.These patients may first present to an ophthalmologist.So we need to be familiar with the same to be able to diagnose & manage them. We present a case of a 63 yr male with complaints of discomfort while using his prescribed glasses. No diplopia. H/O lateropulsion noted for past 1 month.On examination,elevation was restricted in BE. Pill rolling tremors, Romberg test & other cerebellar signs were positive. Diagnosed with BE Supranuclear Palsy; MRI was advised(showed disproportionate midbrain atrophy with concavity),suggestive of PSP.He was referred to neurologist for further management.PSP is a group of Parkinson’s Plus Syndrome/Atypical Parkinsonism.PSP includes death of specific volumes of brain,with disproportionate midbrain atrophy & has poor prognosis. There is no specific treatment for PSP as of now, but by diagnosing early, starting neurologist treatment & rehabilitation, life quality of the patient can be improved.
