Abstract

Nonarteritic anterior ischemic optic neuropathy (NA- AION) is the most common ischemic optic neuropathy, with an incidence rate of 2.5-11.8 per 1,00,000 cases in men older than 50 years. We report here a case of a 52-year-old male with diminution of vision in his left eye which was initially diagnosed to be a case of normal tension glaucoma (NTG). However, marked pallor more than cupping led us to a battery of investigations especially carotid angiography which revealed attenuated internal carotids on the side of involvement with a small eccentric hypodense plaque in the common carotid. It is imperitive to diagnose and treat these cases in time. Carotid angiography can help prevent disastrous CVA like ailments in NAAION patients while helping prevent the loss of vision in the other eye. Meticulous workup of this case that helped in solving the hidden puzzle, can be informative for treating clinicans.
