Abstract
Granulomatosis with polyangitis (GPA) is a multisystem autoimmune granulomatous inflammatory disorder affecting lungs,kidneys,eyes,GIT,ears,nose,CNS, heart.Common ophthalmic manifestations are orbital inflammation,conjunctivitis,episcleritis,scleritis,keratitis,sclerouveitis,nasolacrimal obstruction,retinal vasculitis and rare manifestation like optic neuritis.This is a case report of 30years old female,known case of GPA,on immunomodulators discontinued treatment, presented with painful defective vision in left eye since one month.On examination vision in RE-6/6,LE -6/36 with relative afferent pupillary defect,LE fundus showing hyperemic,blurred disc margins suggestive of optic neuritis with OCT left eye showing inferotemporal thinning of nerve fibre layer with edema in all other quadrants with severe dry eye in both eyes. She was started on high dose corticosteroids following which her vision got improved to 6/6 and she was advised to adhere to treatment and regular followup.
