Abstract

Papillary thyroid carcinoma (PTC) is the commonest neoplasm of thyroid with usual slow growing indolent course. But aggressive metastatic form though rare can follow stormy course Distant metastasis usually involves lungs and to a lesser extent bone, brain and soft tissues, rarely to the orbit. We report an uncommon case of orbital metastasis of PTC presenting with proptosis and hemiplegia in a 61 years- old woman. Chest x-ray showing canon ball lesions, MRI revealed pathological vertebral fracture with compression leading to hemiplegia with TSH level of 2.2 mIU/L and 1.95 ng/dl free T4 level. Fine needle aspiration cytology from thyroid and orbit revealed follicular variant of papillary thyroid carcinoma. While undergoing palliative chemotherapy she succumbed to COVID. Review of literature proved its rarity due to proptosis being presenting feature. Conclusion: Although rare, thyroid carcinoma should be suspected in orbital metastasis following routine clinical algorithm
